Currently insured physicians, medical groups, and ancillary staff may request a copy of their current certificate of insurance by calling our office at 800-553-9293. The receptionist will forward the call to the appropriate agent or customer service representative for processing according to HIPPA standards. Requests may also be made in writing by faxing the request to us at 415-506-3031. Please include the physician or ancillary name and policy number.
